Disclosure Form


 

    • If you have copies of any X-rays or MRI test results, please bring them with you as well.

    • Please be sure to wear comfortable, loose fitting clothing (no jeans or sweats). Women can wear loose fitting jersey type pants and top, bathing suite, or underwear (no under-wire bra please). Men can wear loose pants or shorts or underwear during the treatment.

    • A new client exam, lasts approximately one hour and 15 minutes. The consultation includes an intake, analysis, and treatment.

    • Follow-up visits are approximately 45 minutes

    • Payment is due at the time of the visit, and we accept cash or check

    • Kindly give at least 48 hours notice for cancellations and changes. Full payment is expected for missed appointments.
